Medical Examination
Dr. Haki Laho and Dr. Ivi Kasimati are USCIS-designated civil surgeons, authorized to perform the immigration medical exam and certify Form I-693. Form I-693 reports results of a medical examination to U.S. Citizenship and Immigration Services (USCIS). The USCIS requires the immigration medical examination report to establish that applicants who are applying for adjustment status to become a lawful permanent resident, are not inadmissible to the United States on public health grounds. Medical examinations and vaccinations in the United States are typically required for anyone who files Form I-485, Application to Register Permanent Residence or Adjust Status. Frequently asked questions
Will my health insurance cover the cost of the USCIS Immigration Medical Examination?
We do not take health insurance at 3D Medical Immigration. All costs will be out-of-pocket. Most health insurances will deem it unnecessary for health purposes as this is for immigration purposes. You are welcome to contact your insurance to see if they can reimburse you for these services. Payment is due on the day of your medical exam. We will provide you with a receipt that day.
What is the cost?
The starting price is $370. It is dependent on the person’s vaccination/immunization record. $200 includes the medical exam and certification of the I-693 form and a copy. $170 includes mandatory labs tests of TB, gonorrhea, and/or syphilis.
In addition, titers for MMR and Varicella (these will test for immunity) may be required depending on the person’s vaccination record.

Vaccines are also a separate cost from the $370. You are welcome to get vaccines from your PCP or local pharmacy with health insurance. If you would like to pay out-of-pocket for vaccines, we administer Hepatitis B, MMR, TD, and the Flu vaccines.

What vaccines are required?
Vaccines are determined by the person’s age. Tdap/TD, Hepatitis B, MMR, Varicella, Covid-19, and Flu vaccine are the most common for adults.
