3D Medical Immigration has two male certified civil surgeons designated by USCIS to conduct medical exams for immigration (I-693 form), Ivi Kasimati, MD and Haki Laho, MD.

We understand the importance that the completion of Form I-693 has for you and your family. What happens at the medical exam?

A regular physical exam is done. The doctor will evaluate your vaccination record and determine which vaccines are required for your age. Be prepared to give blood for TB and syphilis testing, and a urine sample for gonorrhea testing. It is our policy to have these tests done by our doctors even if you have had them done previously somewhere else. TB testing is required for ages 2 and over.

Syphilis testing is required for ages 18–44. Gonorrhea testing is required for ages 18-24. Please do not fast prior toyour appointment. It is recommended to eat food and drink water, as it will not affect the test results.

What do I need to bring to the medical exam?

  • A valid photo ID (driver’s license / passport, government issued)
  • Any vaccination / immunization records or proof of immunity (Certified Translation into English in case in a different language)
  • Any medical history regarding POSITIVE TB
  • All the forms that will be sent from our office to your email
  • Interpreter if applicant does not speak English
